A copy of the individualized medication form is available for download here. This form will provide greater confidentiality and only require that you complete it once for up to two weeks of medications needed. You will find the blank forms on the file cabinet to the right when entering the office. Once the form is completed please place it in the mailbox above the cabinet marked “Medication Forms.” Place your child's medication contained in a zip-lock bag, labeled with his/her name, into the appropriate drawer in the glass-covered cabinet to the right of the director's desk in the school office. Please ask your child's teacher or the teacher who opens the school where to place those medications. Then each day please note in the comment section of the daily sign-in sheets: “(Your child's name) needs medications today.” Once the required medication has been administered, the form will be placed in your child's folder until the end of the year. Please let us know if you should have any questions.
Here is a form to update your contact information as we have noticed that several work numbers etc. have changed. Even if you think there has been no change of your information please take the few minutes required to complete the form. We need to be assured that we would be able to reach either you or an emergency contact easily.
